P.131 Acute subdural hematoma in obstetric patients following epidural anesthesia

Background: Post-dural puncture headache is a known complication of neuraxial anesthesia and acute subdural hematoma has been associated with unintentional dural puncture. Notably, significant number punctures remain unidentified. We present two contrasting cases (SDH) following labour epidural no sign at time anesthesia. Patient 1 presented post-partum day (PPD) six, SDH was identified PPD23, blood-patch treatment performed PPD35. 2 immediately, had PPD3, placement PPD4. Methods: The are the literature reviewed for prior cases. Results: developed bilateral severe right abducens palsy. Headache resolved post blood patch however surgical evacuation required palsy persisted. experienced resolution symptoms days patch, small unilateral managed conservatively, neurological deficits. Conclusions: common in postpartum period. majority primary etiology benign. rare but serious anesthesia, consequences may be devastating. Rapid identification early essential to avoid sequelae.
